The mosaic above the arch was commissioned by Pope Sixtus III when he built the basilica in the 5th century. This was after the Council of Ephesus in 451, which proclaimed Mary as Theotokos (Mother of God). This mosaic is part of the overall cycle of the nave mosaic panels, being part of the same commission.
